POSITION: RN Field Nurse

ORGANIZATION: Jordan Health Services Medicare

REPORTS TO: RN Clinical Team Manager or Administrator


The RN FieldNurse is responsible for the timely performance,supervision, training, and documentation of clinical service delivery to thepatient and his/her caregiver in accordance with accepted standards of nursingpractice and regulatory guidelines. S/he assists the RN ClinicalTeam Manager in improving the clinical and financial performance of thelocation.


I. Work Flow Necessary to Ensure Clinical & FinancialSuccess(component proportion 75%)

  1. Reports availability routinely, preferablyweekly, to the scheduler.

  2. Embraces and adheres to the use of electronicdocumentation via the organization s software solution.

  3. Completes assigned Work Flow tasks withintimeframes allotted by the Division.

  4. Responds timely to patient assignments and also respondsin accordance with Divisional guidelines when s/he will not be able to acceptassignments.

  5. Performs and documents nursing service deliveryrelated to patient admission and recertification including the OASIS data setand Plan of Care.

  6. Completes OASIS data set information, asassigned, regarding patient events, i.e. Transfer to In-Patient Facility, DeathIn Home, Resumption of Care, Discharge, and Follow-Up.

  7. Obtains and documents physician orders, whenappropriate.

  8. Responsible for the submission of all documentationand receipt of physician orders to ensure timely locking of OASIS and timelysubmission of billing.

  9. Submits nursing orders and documentation, whenappropriate.

  10. Reviews data regarding patient assignments includingbut not limited to OAISS, Plan of Care, and Coordination Notes prior to servicedelivery.

  11. Submits request for new supply requisitions whena new supply order is needed.

  12. Responds to requests from location staff toclarify information needed for the Plan of Care (485) or OASIS.

  13. Documents OASIS changes in the Coordinationnotes, when applicable.

  14. Submits request to Add New Facility Event, asappropriate.

  15. Assists with intake of patient referrals and incoordination of services between disciplines and other healthcare providers.

  16. Participates in the reporting of lab resultsand/or abnormal findings to the appropriate healthcare professional, physician,and/or field clinician.

  17. Provides supervision of home health aides andLVN/LPN team members, as assigned.

  18. Participatesin patient case conferences and documents ongoing Coordination of Care with other members of the healthcare team.

II. QAPI Activities/Home Health Compare Reports(component proportion 10%)

  1. Reports and documents Client Complaint andClient Occurrence Events, when appropriate.

  2. Participatesin the QAPI process of the agency, seeking to identifyopportunities for performance improvement.

  3. Reports significant patient findings to the RNCase Manager and the RN ClinicalTeam Manager and collaborateson actions or interventionsto be taken.

  4. Reports and documentspatient infections per policy.

III. Program AdministrationCoordination(component proportion 15%)

  1. Responsible for the appropriate use of agency resourcesincluding, but not limited to, supplies and personnelto ensure financialsuccess.

  2. Shares on-call responsibilities, as scheduled.

  3. Works to maintain positive, collaborative relationships with patients, caregivers, referral sources,and/or physicians.

  4. Maintains availability atall times during his/her scheduled hours of work, either in person or by telecommunications.

  5. Maintains current knowledge of nursing practiceand is responsible for personalcontinuing educationrequiredby the BNE.

  6. Participatesin internal staff meetings and trainingactivities, as assigned.

  7. Collaborates with members of the marketingteam to assist in identifying potential referralsources, and patients appropriate for home care.

  8. Performs all other duties as assigned.

. Education

In accordancewith licensing regulations, must meet the following criteria:

Must be a Registered Nurse (RN) licensed in the state of practice or in accordance with the Board of NurseExaminers rules for Nurse LicensureCompact (NLC).

  1. Minimum of two (2) years of active experience as a RN, preferred.

  2. Possession of a working knowledge of Medicare/Medicaid home health care benefits, policies and procedures preferred.

  3. Understanding of Medical Diagnosis Coding,preferred.

  1. Has a positiveattitude and willingnessto learn throughout employment.

  2. Self-starter with excellentverbal and written communicationskills.

  3. Ability to work with both clinical and non-clinical staff members.

  4. Willingness to learn and utilize software solution, inclusiveof Divisional Work Flow Tasks.

  5. Must be organized and have the ability to manage multiple tasks & priorities at any given time.

  6. Possesses a firm foundation of basic nursing skills.

  7. Willingness to learn home health regulatory standardsrelated to job-specific duties, including but not limited to: OASIS, Conditionof Participation,state-specific licensing rules, diagnosiscoding, and internal policies.

A. Musthave satisfactory references from previous employers.

B. Must be able to pass a background check which may include criminalhistory check and OIG sanctionscreening.

C. Mustbe willing to submit to and be able to pass a random drug screening.

D. Must have a dependable vehicle, a valid driver s license, and maintain vehicular insurancein accordance with laws of the State.
